Student COVID Contact & Case Reporting Form

In lieu of parents having to call the school office to report details of any close contact exposures or positive cases for their children, we have created an online form to help streamline the process as well as ensure we are getting the information needed. Only the Principal and Assistant Principals have access to the information submitted on this form.
 
If you were contacted by the school, notifying you that your child was a close contact, you do not need to complete this form. However, if your child subsequently tests positive for COVD-19, please submit this form.
 
The school will follow up regarding any quarantine or testing questions and information.
